On Saturday November 23, the People’s Health & Wellness Clinic will be holding a Vermont Health Connect Sign-up Day at St. Leo’s Hall, 109 South Main St., Waterbury. Certified Navigators will be on site to provide face-to-face assistance to help people compare and select a health insurance plan. Hour-long slots will be available from 9am to 3:30pm.
You must call for an appointment at 479-1229 – there are no walk-ins. People should be prepared to bring last year’s tax return (2012) if they filed, their most recent month’s pay stubs, social security numbers for all household members applying, and a list of any current medications to aid in choosing a plan that covers them.
The event is open to anyone who needs assistance understanding the health insurance options and filling out the application. This is not an outreach event, but face-to-face assistance filling out the on-line application. Navigators will have separate spaces to respect individual and family privacy.
The People’s Health & Wellness Clinic provides primary health care and wellness education for uninsured and under-insured residents of central Vermont who cannot otherwise these services. Vermont Health Connect Navigation assistance is available free to all individuals, families, and small businesses regardless of whether or not they use other PHWC services.
